The form below will generate a Return Authorization number for PSQ-20 and PSQ-20A asset repair. All fields must be populated, except “Address Line 2” fields. Once completed, click “Submit”. An email will be sent to all addresses on the form. Shipping and paperwork requirements will be included in the email. 

In the event an incorrect submission is made, fill out the form with the correct information and re-submit; the last submitted form will be processed and all others ignored.

Accurate information on the DD1348-1A and/or DD1149 is critical, incorrect documentation will hold up goggle processing until resolved.
