The form below will generate a Return Authorization number for PSQ-20 and PSQ-20A asset repair.   All fields must be populated, except “Address Line 2” fields.  Once completed, click “Submit”.  An email will be sent to all addresses on the form along with other appropriate contacts for the repair.  Continue to follow the instructions in the email. 

In the event an incorrect submission is made, fill out the form with the correct information and re-submit; the incorrect form will not be processed.  Accurate information on the 1348-1A is critical.
